1. Who We Are & What This Policy Covers

This privacy policy explains how Resolve Physical Therapy (“we,” “us,” or “our”) handles information collected through our website at resolveptbend.com. We are a physical therapy clinic located in Bend, Oregon.

This policy applies only to this website. It is a website privacy policy, and it is separate and distinct from the clinic's HIPAA Notice of Privacy Practices — the formal notice that governs your protected health information (PHI) as a patient. Any medical records, treatment information, and clinical details you share as a patient are handled under that separate Notice of Privacy Practices, which you receive when you become a patient, not under this website policy.

2. Information We Collect

Contact form. When you fill out the contact form on this site, we collect the information you choose to provide: your name, email address, phone number, the service you're interested in, and the message you write. We ask that you do not include medical or symptom details in the message — your therapist will discuss those with you privately. The form is intended for general inquiries and scheduling, not for sharing health information. If health details do end up in a message, we treat them with the same confidentiality we bring to all patient communications.

Your submission travels directly from this website to our clinic email inbox — no third-party form-handling company receives it. Section 4 below lists exactly which providers are involved along the way.

Website analytics. We use Cloudflare Web Analytics to understand general website traffic, such as which pages are popular. It is designed to be privacy-first and cookieless: it sets no cookies, does not fingerprint your device, and does not follow you across other websites. We see only aggregate statistics — never a profile of an individual visitor.

Basic technical data. Like nearly every website, the infrastructure that serves this site briefly processes standard technical information — such as your IP address and browser type — to deliver pages and to block spam and abuse. We do not use this information to identify or profile visitors.

What we do NOT use. This website does not run advertising trackers, a Meta/Facebook pixel, or Google Analytics. We deliberately keep third-party tracking off this site.

3. How We Use Your Information

We use the information you submit through the contact form to respond to your inquiry, to help arrange consultations or appointments, and to keep a record of our correspondence with you. Technical data is used only to keep the site running and free of spam. That's it.

We never sell your information, and we never use it for advertising.

4. Service Providers

We rely on a small set of service providers to operate this website and communicate with you. Here is who they are and exactly what each one touches:

• Cloudflare — hosts this website and runs the small piece of clinic-owned code that forwards your contact-form submission to our email. Cloudflare handles submissions only in transit: it does not store them, and its logs do not contain what you typed. Cloudflare also provides the cookieless analytics described above.
• Google Workspace — our clinic email system. Contact-form submissions arrive as email in our clinic inbox and stay there. Our Google Workspace account, including Gmail, is covered by a HIPAA Business Associate Agreement (BAA) with Google.
• Prompt — our electronic medical record (EMR) and scheduling system. Booking buttons on this site open Prompt's scheduling page in a new tab; anything you enter there goes directly to Prompt's system, never through this website. Our relationship with Prompt is governed by a HIPAA Business Associate Agreement.
• Google Maps — the map on our contact page is an embedded Google Map. It loads only when you scroll to it, and when it loads, Google receives the standard technical data that comes with displaying a map (such as your IP address). Google's use of that data is governed by Google's own privacy policy.

If we change providers, we will update this policy to match.

5. Cookies

This website itself sets no cookies — no advertising cookies, no tracking cookies, and our analytics run without cookies. The only embedded third-party content on this site — the Google Map on our contact page — is delivered by Google and may use Google's own cookies inside that embed, under Google's privacy policy.

Because this site does not track visitors, browser signals like “Do Not Track” and Global Privacy Control do not change anything here — there is no tracking to opt out of.

6. Data Retention

Contact-form submissions live as email in our clinic inbox. We keep them only as long as needed to respond to your inquiry and in line with the clinic's ordinary records-retention practices. Website analytics are aggregate only and are never tied to any individual.

7. Your Choices & Rights

For information you've submitted through this website, you can contact us (see the details below) to ask what we have, ask us to correct it, or ask us to delete it. We honor these requests as a matter of practice, whether or not a particular privacy law requires it at our size.

Your rights regarding your medical records as a patient — such as the right to access or amend them — are handled separately, under the clinic's HIPAA Notice of Privacy Practices, not through this website policy.

8. Security

This website is served over HTTPS (an encrypted connection) everywhere, and contact-form submissions are encrypted in transit from your browser to our inbox. The systems where sensitive information actually lives — our clinic email and our EMR — are covered by HIPAA Business Associate Agreements with those vendors. No method of internet transmission is ever completely secure, which is one more reason we ask you to keep health details out of the contact form.

9. Links to Other Sites

This website links out to other services — our online scheduling opens in a new tab in Prompt's system, and we also link to our Facebook and Instagram profiles and to our Google reviews. Once you leave this site, the privacy policy of that other service applies, not this one. We encourage you to review the privacy practices of any site you visit through our links.

10. Children's Privacy

This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this site. If you believe a child has submitted information through our contact form, please contact us and we will delete it.

11. Changes to This Policy

We may update this policy from time to time — for example, if we change a service provider. When we do, we'll revise the “Last updated” date at the top of this page, and if a change is significant, we'll note it prominently here. Please check back periodically to stay informed.
